چکیده

BACKGROUND Prostate cancer (PCa) is the most commonly diagnosed malignancy in men and the second leading cause of cancer death in developed countries. Despite the primary treatments, 20-30% of patients experience a recurrence. The main objective of this study was to evaluate the clinical efficacy of salvage HIFU after radical prostatectomy in terms of biochemical free survival rate (BFSR) and PSA nadir. METHODS Twenty two patients with local recurrence of Pca after radical prostatectomy underwent HIFU as first-line salvage therapy. Considering that In all HIFU experience, PSA nadir and PSA failure are different and PSA definition of BFSR is unknown, we defined Treatment success as a PSA nadir ≤ 0.4 ng/ml 3 months after treatment. All early and late medical and surgical complications were recorded. RESULTS Ten of the 22 patients (45.5%) were classified as "success" three months after HIFU, showing a nadir PSA ≤ 0.4 ng / ml; 12/22 patients (54.5%) were classified as "failure" during follow-up (median follow-up: 48 months). Seventeen of 22 (77%) patients were continent (no-pad) before HIFU. A new diagnosis of stress urinary incontinence was made in 5 cases (early onset) after treatment. A case of vesicoureteral anastomotic stenosis was treated, endoscopically through cold urethrotomy. We didn't observe cases of recto-urinary fistula or persistent lower urinary tract symptoms. 2/7 patients, complained about de novo erectile dysfunction After HIFU. CONCLUSIONS The positive oncologic outcomes in the short term anyway obtained in selected patients, associated with documented mild side effects, represent the basis to start more organic, prospective, randomized and multicenter study protocols, that with a long term follow-up could confirm these promising preliminary results.

Robotic Assisted Laparoscopic Prostatectomy after High Intensity Focused Ultrasound Failure

Background. Prostate cancer is the most common cancer diagnosed in men. As new focal therapies become more popular in treatment of prostate cancer, failure cases requiring salvage therapy with either surgical or other techniques are being reported. Objective. To report the options in treatment of prostate cancer after recurrence or failure of the primary treatment modality. Methods. We report a...

Salvage HIFU for biopsy confirmed local prostate cancer recurrence after radical prostatectomy and radiation therapy: Case report and literature review.

High-intensity focused ultrasound (HIFU) is a treatment option for low- and intermediate-risk prostate cancer and more recently has been used as salvage therapy after failed radiation therapy. We present a case of local recurrence with biochemical failure after radical prostatectomy and salvage external beam radiation therapy with salvage HIFU without biochemical recurrence at 20 months.

Perioperative Complications and Long-Term Outcome of Salvage Radical Prostatectomy Compared to Primary Radical Prostatectomy

Material and methods: 36 patients with local recurrence of prostate cancer (PC) after primary curative EBRT, BT or HIFU underwent open salvage rPx. Preoperative imaging was done by choline PET or PSMA PET-CT to check for a possible systemic disease as cause of biochemical recurrence (BCR) in all patients. We evaluated peri and post-operative complications and compared these to open rPx as prima...

Salvage Therapy Options for Local Prostate Cancer Recurrence After Primary Radiotherapy: a Literature Review.

PURPOSE OF REVIEW While recurrence after primary treatment of prostate cancer (PCa) is not uncommon, there is currently no consensus on the most appropriate management after radiation treatment failure. This article seeks to explore the currently utilized modalities for salvage treatment for radiorecurrent PCa. We focused our review on the oncologic outcomes and reported toxicity rates in the l...

Salvage local therapy for radiation-recurrent prostate cancer – where are we?

INTRODUCTION Prostate cancer is the most frequent cancer among males in Europe and a leading cause of cancer deaths, with similar proportion in other developed countries. For more than twenty years, external-beam radiation therapy, alongside with radical prostatectomy, has been used as a primary radical therapeutic approach for localized prostate cancer. Yet, EBRT failures relate to 22-69% foll...
